Is this only for the score at 90 minutes? If not, why is there an option to bet on draw?

In official football terms:

in a knockout phase like this, a draw is counted when the match finishes on even score (like 0-0, 1-1 or 2-2) after normal time (90 minutes) and extended time (30 minutes).

So if this matches finishes with 1-1 after extended time it's called a draw. And it doesn't matter which team wins the penalties: it will be still called a draw.
