I'm not too sure, I think it means the 49ers have to win by at least three points in order for them to win the bet.
I'm not really sure what happens if they win by exactly three points though...

If the difference is exactly 3 its called a "push" and everyone gets their money back, so the 49ers have to win by more than 3.
