I actually think with the time off to recover and the physical training he did (when he did a bit of boxing) Flintoff is probably in pretty good shape physically. I can see him struggling with the rigours of the game though, i.e. bowling in the right areas and at good pace as well as facing bowlers that he won't have seen the likes of for 5 years.
Although saying that, it wouldn't surprise me if he never plays and Lancashire just use his name to sell a lot more tickets.
